Creating a Patient

Create a patient in the Patients window to keep their demographic and contact information together in Phoenix. Start with the patient’s name and date of birth, add the information your practice needs, and select Save Patient.

Before You Begin #

Have the patient’s first and last name and date of birth available. Gather their address, phone numbers, email address, and emergency-contact details if you plan to enter them during registration.

Check whether the patient already has a record before adding another one. In the Patients window, the magnifying-glass button on the upper-left toolbar opens the patient lookup.

Create the Patient Record #

1. Open Patients and Start a New Record #

  1. Select the Patients tab on the Phoenix ribbon.
  2. Select Patients in the Demographics group.
  3. Select Add Patient at the upper right of the patient window. Phoenix clears the demographic fields for the new entry.

If Phoenix asks whether to save changes to the current record, choose Yes to save those changes first, No to continue without saving them, or Cancel to stay on the current record.

2. Enter the Patient’s Name and Date of Birth #

  1. In Patient Demographics, enter Last Name and First Name.
  2. Enter the patient’s Date of Birth. Check the complete date before continuing.
  3. Add a Middle Initial, Nickname, or Salutation when appropriate.

For this workflow, leave the patient-code box on the upper-left toolbar blank. Phoenix can assign a code when you save the patient with their name and date of birth entered. The patient code identifies the patient record; Case Quick Code, farther down the form, identifies a case.

3. Complete Demographic and Contact Information #

Enter the information available for the patient. Use the following areas as a guide:

You can save a basic patient record before entering address, phone, or insurance information. Complete the additional details your practice needs as part of its registration workflow.

4. Save the Patient #

  1. Review the patient’s name, date of birth, and any other information entered.
  2. Select Save Patient at the upper right.
  3. If a Save Failed message appears, select OK, complete the information it requests, and select Save Patient again.

What Happens After Saving? #

The saved patient remains open, and the patient code appears on the upper-left toolbar. Save Patient becomes unavailable when there are no further changes to save. Phoenix also supplies a Case Quick Code for the initial case.

To find the record again, open the patient lookup with the magnifying-glass button on the upper-left toolbar. The saved patient appears in the list.

The Case section below the contact information contains separate 1. Primary Insurance, 2. Secondary Insurance, and 3. Tertiary Insurance tabs. Creating the basic patient record does not require completing those insurance fields. Enter case and coverage details as part of the patient’s billing setup.

If Phoenix Will Not Save #

Phoenix identifies missing information in a Save Failed message:

Correct the item named in the message, then save again. Phoenix may ask for another missing item after the first one is corrected.
