Subscriber Information #
The subscriber is the person under whose insurance coverage the patient is enrolled. The subscriber may be the patient, but that is not always the case.
Before you begin #
- Confirm the patient and insurance policy you are reviewing.
- Use the insurance card or another approved source.
- Do not assume the subscriber is the guarantor.
What to do #
- Open the patient’s case and review the relevant insurance section.
- Identify the subscriber information associated with that policy.
- Enter or correct the subscriber details supported by your source.
- Review the relationship-to-subscriber field separately.
- Save the patient record.

Verify before you finish #
- The subscriber belongs to the correct policy.
- Subscriber information is not being copied from another coverage without verification.
- The relationship to subscriber matches the patient’s actual relationship.
Important
Subscriber, patient, and guarantor are different billing concepts. They may point to the same person in some accounts, but Phoenix users should verify each role separately.
