Coverage Effective Dates #

Coverage effective dates record the period associated with a patient’s insurance information. Dates stored in Phoenix should reflect the information available to your practice, but stored dates alone do not prove current benefits.

Before you begin #

  • Verify the patient and policy.
  • Use the coverage information available from the payer, card, or another approved source.
  • Check the date of service when investigating a billing issue.

What to do #

  1. Open the patient’s case and insurance section.
  2. Review the coverage dates associated with the selected policy.
  3. Enter or correct the applicable effective or termination dates when supported by your source.
  4. Confirm the dates belong to the correct coverage level.
  5. Save the patient record.

Verify before you finish #

  • Start and end dates are not reversed.
  • Dates belong to the intended payer and policy.
  • The date of service falls within the period you intended to record.

Important

Stored dates are not a substitute for eligibility. When current benefits matter, use the approved eligibility workflow and review the payer’s response.
